Saint Anne's Preschool

Based on observation, the licensee did not comply with the section cited above. LPA observed Lysol Disinfectant Spray and Clorox Disinfectant Wipes on the kitchen counters, in the restroom, and in both classrooms on accessible counters. LPA also observed ZEP cleaning solution and a wash rag hanging from the sink located in the dining room which poses an immediate health, safety or personal rights risk to persons in care.

Based on observation, the licensee did not comply with the section cited above. During today's inspection, LPA observed that the blankets for all of the children were wrapped up together and stored on top of the napping cots. The children were also observed sleeping on the napping cots without sheets which poses a potential health, safety or personal rights risk to persons in care.

Based on observation, the licensee did not comply with the section cited above. LPA observed several toys scattered on the floor near two trashcans in the dining room. There were supplies stacked in totes with missing lids leaning against a storage rack. The restroom floors were sticky and debris under the changing table pad. A play kitchen contained an unknown substance in the sink and a soiled rag laying on top. LPA observed other play houses that were covered in dust and contained bird droppings/ feces which poses a potential health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above. As of 02/27/2025, per Guardian Employee Roster, Staff #1 is fingerprint cleared but not associated to Saint Anne's Preschool which poses a potential health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above. The facility was unable to produce proof of current mandated reporter training for Staff #1, Staff #2, and Staff #3 which poses a potential health, safety or personal rights risk to persons in care.

This requirement was not met as evidenced by interview with Director Lopez indicating that the required drinking water testing has not been completed. This poses a potential threat to the health, safety, or personal rights of children in care.

This requirement was not met, as during records review, LPA found that Staff #1's file did not contain proof of the required educational credits. This poses a potential risk to the health, safety, or personal rights of children in care.

Based on record review, the licensee did not comply with the section cited above in 6 out of 6 children's files reviewed which poses a potential health, safety or personal rights risk to persons in care.
