Based on [(observation) (interview) (record review)], the licensee did not comply with the section cited above in staff did not have a current mandated reporter training certificates which poses/posed a potential health, safety or personal rights risk to persons in care.
Based on observations, and interviews, children stated they sometimes have to wait a little while until being done with an activity to drink water and are not able to drink as they wish.
Based on observation, interview, record review, the Director did not comply with the section cited above 3 out 4 of staff files reviewed shows staff #1 and #4 have a expired mandated report on file and Staff #2 Director was unable to produce a copy of currently mandated reporter training which poses/posed a potential health, safety or personal rights risk to persons in care.
