Based on interviews and record review, the Licensee did not ensure C1 was provided a safe environment when C1 was bitten four times by C2 between August 2023 and October 2023.

Based on interviews, C1, C2, and C3 wandered outside of the enclosed play area without S1, S2 or S3 observing. This posed an immediate risk to the health and safety of children in care.

Based on record review, the licensee did not comply with the section cited above in eight of ten child files reviewed. This poses/posed a potential health, safety or personal rights risk to persons in care.

Based on observation, the licensee did not comply with the section cited above when LPA observed Tide, carpet cleaner, floor cleaner, and dish soap in the closed cabinet under the kitchen sink. The lock on the cabinet was not working at the time of this inspection and the chemicals were accessible to the children in care, this posed a potential health, safety or personal rights risk to persons in care.
