Based on observation, the licensee did not comply with the section cited above as their was a partition removed in classroom 2's bathroom that was not mentioned to the department which poses a potential health, safety or personal rights risk to persons in care.
The Site Director shall submit the written report (LIC624) for the child's injury to the Department by 3/29/2024. LPAs advised the LIC624 may be emailed or faxed to the Department once completed. The Licensee did not submit a written report or make an initial report within 24 hours for an injury that occurred to a child that required medical treated, which poses a potential risk to the health, safety, and personal rights of children in care.
The Site Director shall submit mandated reporter training certificate for one staff member by 4/8/2024. The Licensee did not ensure that all staff members completed their mandated reporter training as one staff members expired in 2023, which poses a potential risk to the health, safety, and personal rights of children in care.
