The facility took steps to remediate the hand washing fixtures by installing lead filters and retesting the fixtures on 8/30/2023. The fixtures then tested below the ALE level and are now in compliance. Based upon record review, the facility had two fixtures (E and H) that tested above the Action Level of 5ppb. This posed a potential health, safety, or personal rights risk to persons in care. 8 This deficiency will be cited and cleared the same day of the visit, 11/7/2023.
