By Plan of correction due date, 06/27/2025, Licensee will submit the Copy of training material of Supervision policy and copy of the attendance sheet of all the attendees regarding the training on Supervision policy for all staff to the department. Site Supervisor self-admitted that the child (C1) was left in the library, without any teacher being present for about an hour and half as child (C1) was sleeping in the library(Room 16) during morning after child was dropped off at the facility, on May 12, 2025 and based on interviews, four(4) children(C2, C3, C4 &C5) were left unattended in the bathroom on May 30, 2025, which poses an immediate risk to the health, safety and personal rights of children in care.

Staff removed the items immediately during the inspection and placed them in an area inaccessible to children. Deficiency corrected. On May 20, 2025 LPA inspected classrooms for a complaint investigation and observed disinfecting wipes in classroom 1 within reach of children. This poses a potential health and safety risk to children in care.

During today's inspection LPA Macias requested the following documents; Self-Certification LIC9275, Sampling Checklist Form LIC9276, Facility Sketch LIC 999, fully labeled with locations of all water outlets, and full lead report. Sample D exceedance level is 5.7. This poses a potential risk to the Health, Safety, or Personal Rights of children in care. 8 The Director will identify the location of the lead exceedance and ensure children are unable to access the water source. The water source will be labeled and or shut off. The Director will submit a letter outlining the drinking water procedure by the POC date.
