To receive safe, healthful, and comfortable accommodations, furnishings, and equipment. This requirement has not been met by evidence: Facility served milk that expired one day after expiration date. This is considered as a potential risk to the children in care. 1 Headstart manager stated that they have had a training to prevent future incidents from occuring. LPA cleared the deficiency.

No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. 1 Facility management had a meeting with all staff regarding head counting and roll call during transitions. LPA obtained a copy of signed meeting agenda by all staff. This deficiency is cleared as of today. This requirement is not met as evidenced: Facility self reported that Child #1 was left behind in the classroom for approximately one minute during a practice fire drill. This is an immediate risk to the health and safety of children in care. 8
