Based on record review and interviews which were conducted, C1 was left unattended and unsupervised for approximately 40 minutes. This poses an immediate health, safety or personal rights risk to children in care. 8 All teaching staff were retrained on program's name to face, transition procedure and policy. Copy of guidance was provided to LPA during visit. Proof of staff attendance will also be submitted to LPA.
