Based on interview and record review, the facility did not comply with the section cited above as facility self-reported an incident where a toddler age child was left in outdoor playground during a transtion back into classroom for lunch,which poses a potential health, safety or personal rights risk to persons in care. 8 Director will email LPA a copy of training agenda conducted on January 22, 2026. Agenda will include signatures of staff acknowledging attendance and understanding of supervision procedures discussed during training.
