Based on record review, LPAs identified that the incorrect live scan form was used for one of the teachers. This resulted in incomplete criminal record clearance. Therefore, licensee did not comply with the section cited above in one out of three persons which poses an immediate health, safety or personal rights risk to persons in care. This is an amended report. The type A violation has been changed to a type B violation.
Based on results received from the certified water samplers, the facility exceeded 5.5 ppb of lead in their water source. This pose a potential health, safety, or personal rights risk to persons in care.
