Based on interviews conducted with the director, S3, and S1 and records review, the licensee did not comply with the section cited above in one out of one staff, which poses an immediate health, safety or personal rights risk to persons in care. Cynthia Castillo stated she has worked at the facility for 1 year. The director and S3 confirmed Cynthia has been working at the facility since approximately August 2022.
Based on interview and records review, the licensee did not comply with the section cited above in 1 out of one children's roster, which poses/posed a potential health, safety or personal rights risk to persons in care. Per the director, the facility did not have a roster at the time of inspection. Director stated they would have to prepare one on the spot. The LPA provided a blank copy of the LIC9040.
