Based on record review, the licensee did not comply with the section cited above in that Staff 2 (S2) had a mandated reporter training certificate that expired 1/25/2026 which poses a potential health, safety or personal rights risk to persons in care.

LPA Nguyen provided business card with contact information. Base on record review and observation, S1,S2, S3's Mandated reporter training certificate expired. S1's certificated expired 03/27/2018; S2's certificated expired 03/28/2018; S3's certificated expired 08/30/2018. This poses a potential Health and Safety risk to the children in care. 8
