Licensee shall submit a written summary of the steps taken and plan emplemented to prevent another incident from happening by 6/5/2026. interviews and record reviews which revealed during transition to lunch, a child was left unattended in the bathroom for 49 seconds, until discovered by a passing by staff member. This poses a potential health and safety risk to children in care.
