A REVIEW OF RECORDS AND INTERVIEWS WHICH 1 Licensee will install a safety latch onto the gate, that is out of reach of children, to ensure that a child is unable to open the gate and leave the play area. Licensee will install the safety latch by 10/14/25. REVEALED WHILE PLAYING ON THE PLAYGROUND, A CHILD WAS ABLE TO OPEN THE BACK GATE AND RUN ONTO THE STREET.
(a) The licensee shall ensure that each child is accorded the following personal rights: (3) To be free from corporal or unusual punishment, infliction of pain, humiliation, intimidation, ridicule, coercion, threat, mental abuse or other actions of a punitive nature including but not limited to: interference with functions of daily living including eating, sleeping or toileting; or withholding of shelter, clothing, medication or aids to physical functioning. 1 Licensee will conduct an all staff training on child personal rights. Licensee will submit to community care licensing a staff sign in sheet of all in attendance and a summary of the training and changes that will be made to prevent future incidents by 3/1/23. Lpa discussed TSP program and supervision with center director. Should she want to enroll, contact lpa for referral This requirement was not met as evidence by: interviews and self reported unusual incident; in an attempt to keep several children from climbing a bookshelf, a teacher "swat" a child on the forehead when she swung her hand backward. 8
