Based on record review, the licensee did not comply with the section cited above. Per Site Supervisor, the facility was built after 2010; however, there was no proof indicated that the building was built after 2010 which poses a potential health, safety, or personal rights risk to children in care.

Based on observation, the licensee did not comply with the section cited above. During the outdoor inspection, LPA observed the sensory table, the children kitchen aid, and the sand box cover collected rained water that is unsafe and unsanitized to the childrenin care. This poses a potential health, safety, or personal rights risk to children in care.

During the investigation inspection on 02/27/2023, LPA conducted record reviewed and Staff #1 did not have the Mandated Reporting Training for the General Training and the Child Care Providers (AB1207), Staff #2 did not have the Mandated Reporting Training for the General Training and the Child Care Providers (AB1207), Staff #3 did not have the Mandated Reporting Training for the Child Care Providers (AB1207) and Staff #4 did not have the Mandated Reporting Training for the Child Care Providers (AB1207) which poses a potential health, safety, or personal rights risk to persons in care 8 Director stated that Staff #4 no longer employed with the Stepping Stones Preschool as of 03/21/2023. Director will need to submit proof that Staff #4 is no longer an employee. Per Director, she will have the church’s pastor to submit written proof that Staff #4 no longer an employee of the Stepping Stones Preschool. Director agreed to submit written proof to Community Care Licensing-Fresno Office by 04/28/2023.

Based on record review and staff interview, the licensee did not comply with the section cited above. During the course of the investigation, upon staff record review and interview, staff #4 was substituting when a qualified teacher was not available to provide supervision. Staff #4 did not meet the education requirement which poses a potential health, safety, or personal rights risk to persons in care. Director will need to submit proof that Staff #4 is no longer an employee. Per Director, she will have the church’s pastor to submit written proof that Staff #4 no longer an employee of the Stepping Stones Preschool. Director agreed to submit written proof to Community Care Licensing-Fresno Office by 04/28/2023.

Based on record review the licensee did not comply with the section cited above. 1 Director agreed to submit the director packet reflecting licensing required qualifications and forms to CCL-Fresno Office by Plan of Correction (POC) due date 03/13/2023 Director at this facility stated she is the new director; however, this has not been verified as director packet has not been submitted to Community Care Licensing (CCL)-Fresno Office which poses a potential health, safety, or personal rights risk to persons in care.

Based on observation, the licensee did not comply with the section cited above. Licensee was unable to verify if the facility has a working carbon monoxide detector, which poses a potential health, safety or personal rights risk to persons in care.

Adults who provide supervision to day care children do not have the required Mandatory Reporter Training.

Facility is using staff who do not have the required education to provide care to day care children.
