Based on interviews with staff, the facility did not comply with the section cited above as the facility operated out of ratio which poses/posed a potential health, safety or personal rights risk to persons in care. 8 sign this acknowledgment form to confirm that they have received this training and understand their responsibility to maintain ratio at all times. LPA Uribe will return to the facility after the due date of 08/14/2023 to obtain a copy of these signed and dated acknowledgement forms.
