Based on interview and record review, the licensee did not comply with the section cited above as one staff member does not have proof of Measles (MMR) immunization which poses/posed a potential health, safety or personal rights risk to persons in care.
Based on interviews conducted which showed a preponderance of evidentiary statements that staff yell at daycare children which poses a potential health, safety, or personal rights risk to the persons in care. including having video on and participation from all attendees. Director will create an acknowledgement form which each staff member will date and sign stating that they have taken the training in both parts and completed it. This signed form will be emailed to LPA Uribe no later than 5 days after the conclusion of the training.
