The department shall assess an immediate civil penalty of five hundred dollars ($500) per violation and one hundred dollars ($100) for each day the violation continues after citation, for any of the following serious violations:(3) Absence of supervision, including, but not limited to, a child left unattended, and supervision of a child by a person under 18 years of age This requirement was not met as evidence by: 1 The licensee has locked the side entrance gate and will be speaking to the fire marshall for guidlines. Licensee to hold an all staff meeting to discuss active supervision and safety measures. An interview with S5 who stated she was left alone during naptime with a least 20 children; some children were awake while the others were sleepingg. S5 stated additional staff are not available at the facility to assist. Director also agrees to provide a written statement on how she will ensure teacher to child ratios at all times.
