Based on observation, interview, record review, the licensee did not comply with the section cited above in staff (S4) did not have a current mandated reporter training certificate (expired on 11/27/2025), which poses a potential health, safety or personal rights risk to persons in care. Director acknowldeged the certificate was not curent.

This requirement was not met as evidence by Interviwes conducted, LPA interviewed multiple former and current staff. LPA discovered a pattern of behavior from S1, and one staff observed S1 punish and yell at children as a away to make an example of the child in front of other children. In particlualry it was found C8 was not allowed to go to the bathroom by S1. And S1 took food away from children. 8 CCR
Based on interviews conducted, S2 was removing food from children prior to children being done with there meals which poses a potential health, safety or personal rights risk to persons in care.

Resigned on April 6th, 2023. This is met by: S1 acknowledgment of allegation, photographic evidence, and IB investigation. 8
