(a) In addition to the sign-in procedure requirement of Section 101226.1(b), the licensee shall develop, maintain, and implement a written procedure to sign the child in/out of the child care center that shall, at a minimum, include the following: 1 The facility already updated the procedures for the proper signatures from the parents for each time they pick up and drop off. The facility agreed to continue to use the proper procedures. (1) The person who signs the child in/out shall use his/her full legal signature and shall record the time of day. This requirement is not met as evidence by record reviews, LPAs observed the sign in and out had parents' initials only. This poses a potential Health and Safety risk to child in care.
The Licensee shall take mearures to keep the center free of files, other insects, and rodents. This requirement is not met as evidence by LPA's observations of cockroaches and flies in the facility. 1 Licensee will be putting in new floors and the base boards to prevent insects from accessing the facility. Licensee will continue having pest control services from CLARKS on a monthly schedule. This poses a potential Health and Safety risk to child in care. 8
(a)The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. This requirement was not met as evidence by LPAs observations. 1 Director will meet with Licensee and board of director regarding the deficency. LPAs oberved the following throughout the facility; several parts of the vinyl on the floor lifting & exposing the carpet underneath it, exit door with a chipped paint, missing small part of the base board exposing chipped paint, small exposed spot of the carpet, carpet was not clean showing brown to gray stains, the stair there was a missing the vinyl exposing the wood underneath it, and insufficent cushioning of barks undert the play structure. This poses potential risk of the health and safety to the children in care. 8

interviews revealed that a staff member inappropriately grabbed and scooped a child up, as a method of redirection.
(b) The person who brings the child to, and removes the child from, the center shall sign the child in/out. This requirement was not met, evidenced by: LPA observed sign in and out sheets to be incomplete and without a parent signature for present children. Parent interviews revealed that sometimes they forget to sign in and out. 1 Director stated that they will continue to remind parents about the importance of signing the children in and out, and will come up with a plan to ensure parents are signing in and out. Director will submit their plan to LPA Tillery.
(a) The licensee shall provide care and supervision as necessary to meet the children's needs. This requirement was not met, evidenced by: LPAs observed 6 preschool children to be in the preschool room while staff #1 was in the kitchen. 1 Director will conduct a training with staff regarding the requirement of 100% supervision and submit proof to LPA Tillery.

LPAs observed the wooden floor to be missing boards which could be a tripping hazard, which poses/posed a potential health, safety or personal rights risk to persons in care.
