There shall be a ratio of one teacher for every four infants in attendence.This requirement is not met as evidence by LPA's observations of two unqualified staff supervising 7 infants on 8/15/2023 in the classroom. 1 Facility representative will send a statement requesting an exception to have an aide with 9 Child Development Units and currently attending more ECE units including the Infants/Toddler course. This poses an immediate health and safety risk to children in care. 8

Based on observation and interview, the facility did not comply with the section cited above by not using sheets on the nap cots, which poses/posed a potential health, safety or personal rights risk to persons in care.
(b) The person who brings the child to, and removes the child from, the center shall sign the child in/out. This requirement was not met, evidenced by: LPA observed sign in and out sheets to be incomplete and without a parent signature for present children. Parent interviews revealed that sometimes they forget to sign in and out. 1 Director stated that they will continue to remind parents about the importance of signing the children in and out, and will come up with a plan to ensure parents are signing in and out. Director will submit plan to LPA Tillery.
Based on observation and interview, the licensee did not comply with the section cited above that, LPAs did not observse a trash bin with a tight fitted lid. LPAs observed a staff member toss diapers in a bag and take the bag to the outside trash bin, this which poses/posed a potential health, safety or personal rights risk to persons in care.
