Based on observation, interview, the licensee did not comply with the section cited above in which the outdoor activity space needed minor repairs which posed a potential health, safety or personal rights risk to persons in care.

Lead water testing samples taken on 08/18/2022 revealed that levels of lead 1 Per Director fixtures will be replaced with an acceptable faucet by the State of Calfornia on 3/14/2023 which will then be scheduled for a retest. Director will provide retest test results on or before 6/5/2023. were beyond the allowable amount deemed by the State of California and have yet to be retested. This is a type B violation which poses a potential Health, Safety or Personal Rights risk to persons in care.

Director will provide plan of correction and any update as necessary by 12/30/2022. were beyond the allowable amount deemed by the State of California. This is a type B violation which poses a potential Health, Safety or Personal Rights risk to persons in care.
