Based on interviews, the licensee did not comply with the section cited above in that, Child 1 (C1) was in the play yard without staff supervision, which poses an immediate health, safety or personal rights risk to persons in care. 8 regarding the incident which occured on 10/06/2025 and submit all documentation to LPA via email by due date.

Based on observations, interview statements, and records review, the licensee did not meet the regulation above, which poses/posed a potential risk to children in care. The licensee did not provided a copy of a licensing report dated 6/26/24 to parents until 7/1/24 8 The director will email to the LPA copies LIC9224 signed after 7/8/24. These forms were observed to not have a date or signature as of the writing of this report.

Based on interview statements, the reuglation was not met. S2 admitted they grabbed C1 by the hand, held C1 from behind, and picked up C1. S4 admitted she/he threatened C1 to coerce C1 into compliance. This poses an immediate risk to the safety and personal rights of clients.

Building and Grounds (a) The Child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. This requirement not met as evidence by: 1 Facility staff followed Written Directives in PIN 21-21 CCP for lead water testing. Licensing Forms completed; 9275, 9276 and facility sketch submitted. Notification sent out to parents, water outlet flushed (not in use), conditioning for 3 weeks, water filter installed onto pipping, Based on lab report analysis water from faucet E (10.0 parts per billion). These faucets samples taken on 05/06/2022 indicate levels of lead in exceedance. 8 Reusable pitchers of water from the other PASS sinks readily available for the staff and children. Director stated that she will submit new restest results to LPA via email to dianna.valdezsantana@dss.ca.gov. In addition facility provides bottled water for children to drink.

Staff did not promptly give parents a copy of the report dated 6/26/24, which documents one Type A citation.
