(1) No child(ren) shall be left without the supervision of a teacher at any time... 1 Assistant director stated additional training face to name will be provided to staff. Performance and improvement plan has been placed in stafff files. Based on interview by staff the licensee did not comply with the section cited above due to child remaining in the class with no active supervision which poses a potential health, safety or personal rights risk to persons in care. 8

Per Facility, the Administration will submit a plan of correction to the Department within 24 hours. Which include positive redirection training and submit certificate/registration proof with S1 and S2 written summary of understaning of training to the Palmdale RO by due date by email. LPA's observation witnessing (S1) and (S2) did not accord the child/children dignity when (S1) yelled at the (C1)while in care and while other children present. (S2) inappropriately interacted with (C1), (C2), (C3) and (C4). This poses a potential risk to the health and safety of the children in care.

LPA's observation: (S1) and (S2) humiliated/shamed, punished the child/ren in care while other children were present. (S1) yelled "shut your mouth" in close proximity addressing (C1). (S2) stated to multiple children "are you a baby" utilizing shame and used statements such as IF you..I am going to... This poses a potential risk to the health and safety of the children in care. 8
