Based on LPA video observation and staff interviews, the facility did not comply 1 Staff #1 was removed from the facility and a parent meeting was conducted on 8/10/26. with the section cited above in staff #1 grab and lift up C1 from the left arm walked over to C1 cot and place down C1 on the cot. Staff #1 tossed C1 on the cot which poses an immediate risk to the health and safety to the children in care.
Based on record review, and interviews the facility did not provide parent a i 1 Report was given to parent on 8/10/26 after having a meeting. Director will provide parents a incident report the day of the incidents. ncident report on the day 8/7/26, parent was notified verbally which posed a potential health, safety or personal rights risk to persons in care.
