Based on the 1 Manteniance staff immediate arrived at the site and removed the dead rodent and cleaned under the sink area. Faciity will ensure to keep all food iitems sealed. All utensils and serving plates are stored in the tight containers for sanitary purposes. Pest Control had visited the site on 05/02/22. document reviewed, facility had provided reasonable information addressing the pest issue which poses an potential health and safety risk to children in care. 8 Facility representative stated center staff will attend online training regarding health and safety of the facility and submit to LPA by 05/20/2022 in order to clear this citation.
