101238 Buildings and Grounds (a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. This requirement was not met by: 1 On 4/3/2025, before school started, the shelf was bolted to the wall. Director stated she will go through the classrooms and secure shelving. Based on observation, documentation, and interview, the licensee did not comply with the section cited above. The shelf was not bolted when the incident occured, making it unsafe for children present, which poses/posed a potential health, safety or personal rights risk to persons in care. 8
