Based on record review, the licensee did not comply with the section cited above in three (3) out of five (5) staff files which poses a potential health, safety or personal rights risk to persons in care.
Based on observation, the licensee did not comply with the section cited above. While touring the facility at 9:10 a.m. LPA observed liquid Lysol and disinfectant spray cans inside an unlocked cabinet that was accessible to children in care. The items were located next to the childrens games and books inside classroom #4 which poses an immediate health, safety or personal rights risk to persons in care.
Based on observation, interview, and record review, the licensee did not comply with the section cited above. During staff file reviews, LPA observed 4 out of 5 files were missing their mandated reporter certification which poses/posed a potential health, safety or personal rights risk to persons in care.
