On 3/21/24, at approximately 3:42PM, during transition from the Toddler classroom 1 to the playground, Staff 1 did not conduct a proper name-to-face roll call, nor a visual sweep of the classroom, resulting in Child 1 being left inside Toddler classroom 1, unsupervised for approximately 3 minutes. This poses an immediate health, safety, and personal rights risk to children in care. 8 Deficiency was cleared at the time of visit and no further action is needed.
