Based on interviews and staff admissions, the licensee did not comply with the section cited above in that it was determined that on numerous unidentified dates staff left children unsupervised in the classroom, which poses an immediate health and safety risk to children in care.

Based on interviews conducted, the licensee did not comply with the section cited above, as facility staff used a bench, located next to the toilet in the children’s bathroom as a disciplinary tool, in the form of a time-out chair, which poses an immediate health, safety and personal rights risk to children in care.

Based on interviews and admissions, the licensee did not comply with the section cited above in that staff leave children supervised by unqualified teachers during specialty classes which poses a potential health and safety risk to children in care.

Based on observation (at 12:40pm), interview and record review, the licensee did not comply with the section cited above in two (2) out of three (3) medications did not have proper paperwork or prescription label which poses a potential health, safety or personal rights risk to persons in care.

Staff do not provide adequate supervision to day care children.

Staff has children eat lunch in bathroom as a form of punishment.
