Based on record review, the licensee did not comply with the section cited above. S1 did not have proof of MMR (measles) on file for review. Although there was a written statement stating immunity from a licensed physician from another country, Title 22 does not accept health information from any physician, let alone another country for this vaccine. This poses/posed a potential health, safety or personal rights risk to persons in care.

Based on observation, the licensee did not comply with the section cited above in objects which poses a potential health, safety or personal rights risk to persons in care. LPA observed no children in the classroom at the time; however, there were 7 children on the playground. LPA observed the bleach bottle and the air freshener to be in an upper cabinet not immediately available to the children. LPA observed there is a potential risk a child could reach the items if child was to get on a chair. LPA observed the lock to be broken.

AD stated the lead testing was conducted 2/23. AD stated lead was found and will re-test and send the results to the LPA.
