Once the investigation is closed and any other actions are taken- documentation will be provided to LPA. This requirement was not met by evidenced: Based on interviews conducted Staff #1, violated Child's # 1 personal rights. As one staff witnessed and heard Staff # 1 pinching child and asking them if they liked it. This poses an immediate risk for health and safety of children in care. 8
