Based on LPAs observation of one large knife located in Preschool Classroom 12 within children's reach, which is an immediate health, safety, and personal rights risks to children in care.

Based on record review, the licensee did not comply with the section cited above whereas Interim Director does not posess a valid mandated reporter training certifcate from an approved vendor, which poses a potential health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above in that Emily Luchetta and Hannah Buckingham do not have current Mandated Reporter trainings, this poses a potential health, safety or personal rights risk to persons in care.

Based on records review, the licensee did not comply with the section cited above in that Karey Richard, Hannah Buckingham, and Emily Luchetta do not have proof of immunizations on file at the Child Care Center. This poses a potential health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above in that Karey Richard and Emily Luchetta do not have teaching qualifications on file at the Child Care Center. This poses a potential health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above, Ms. Kimberly Raine, did not transfer her Criminal Record Clearance to CCC. She started working with children at the CCC on 9/7/2022 which poses an immediate health, safety or personal rights risk to persons in care.

Based on observation, the licensee did not comply with the section cited above, dring the inspection, it was observed that there were 36 day care children present which poses an immediate health, safety or personal rights risk to persons in care.

Based on record review and interview, the licensee did not comply with the section cited above, Ms. Saldita stated that they conduct fire and disaster drill but it was not documented. which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above, 5 out of 7 staff have no record of immunization on file which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on observation and interview the licensee did not comply with the section cited above, Ms. Soldita stated that former Director Ms. Wishon is no longer working as director. New director was hired however, qualifications were not verified and there are no required documentations for new director on file which poses/posed a potential health, safety or personal rights risk to persons in care. Ms. Soldita stated Kim Wishon is still part of the School, however, the latter was not peresent.

Based on observation and review the licensee did not comply with the section cited above, there is no qualified substitute to act as a preschool director at the time of the inspection which poses/posed a potential health, safety or personal rights risk to persons in care. Based on record review the licensee did not comply with the section cited above, the newly hired director has no documentation on file including the 16 hours of health and training, which poses/posed a potential health, safety or personal rights risk to persons in care.
