LPA Clayton review of information provided by the RP and staff interviews showed that staff S5 yelled at and threatened a child in care which caused the inappropriate interaction with a child in care, which poses an immediate Health and Safety risk to children in care. 8 Admin Hale agreed to review the California Preschool Instructional Network ( https://cpin.us/ ) a professional development system that provides high-quality staff training, instructional resources to early childhood educators to develop a plan of correction to be provided to the department no later than 08/20/2026.

101238 Buildings and Grounds (a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. (1) The licensee shall take measures to keep the center free of flies, other insects, and rodents. This requirement not met as evidence by: 1 Administrator took actions: rodent issue assessed, and is no longer an immediate threat to the children, staff members, and visitors. Pest service inspection completed; entry points repaired, and thirty day trapping plan put in place. Unusual incident report (LIC 624) submitted. Licensee did not ensure that facility was free of rodents. The facility shall be in sanitary conditions at all times. Rodents and rodent dropping presence at the facility was confirmed. This is a Type B violation, which poses a potential health and safety risk to persons in care. 8
