Based on record review, the licensee did not comply with the section cited above as 3 of 10 children's files reviewed by LPA did not have proof of immunization, which poses/posed a potential health, safety or personal rights risk to persons in care.

Director will hold a staff meeting discussing the policies that will be put in place to ensure personal rights of children are not violated. Staff will watch a video on the CCLD website titled: Children's Personal Rights and formulate a plan on how to redirect children without violating personal rights. This requirement is not met as evidenced by a staff member handled a child in a rough manner which poses a potential health and safety risk to children in care. 8 The plan of correction is for Director to submit a list of signatures of all staff who attended the meeting and a meeting agenda. These documents must be submitted to LPA via email no later than 11/14/2022 at Lorraine.Dacanay-Breaux@dss.ca.gov
