Based on record reviews, on 04/23/2024 and 04/24/2024, there were 16 children signed in, but only one staff signed in at the time. S-1 only completed 12 quarter units and was left alone with the children. This poses an immediate health and safety risk to children in care.

Based on record review, S-1 and S-2 were fingerprint on a different database and do not have cleared criminal record clearance on the Community Care Licensing database. This poses an immediate health and safety risk to children in care. 8 Director understands that S-1 and S-2 cannot return until their fingerprints are cleared and associated to facility.

Based on interview, the center did not have a working phone on 04/19/2024. This poses a potential health and safety risk to children in care.

Based on observation, at 10:35AM, a child went to the restroom. There were staff in the room; however, they were not positioned where they can visually supervise the child in the restroom. This poses a potential health and safety risk to children in care.

Based on observation, and interviews conducted, Licensee did not comply with the section cited above. During investigation, it was discovered there was broken/cracked toys, wood planters that can cause splinters, wooden bench around tree that is broken with nails sticking out and sharp corners, which posed a potential health, safety or personal rights risk to persons in care. 8 sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors.

LPA referred Licensee to Title 22 section 101212 (d) Reporting Requirements. Licensee to read section and submit a written statement stating they read and understand the regulation, by POC date 2/12/2024. Licensee failed to report an unsual incident report as required that occured on 10/10/2024 were child (C-1) hurt staff (S-1). This poses a potential risk to the health and safety of children in care.
