Based on interview and record review, the licensee did not comply with the section cited above. A children was left alone and unsupervised in a room for approximately two minutes, which poses a potential health, safety or personal rights risk to children in care.

Based on results of lead testing of water, facility did not ensure to maintain level to be under 5 ppb, which poses a potential Health, Safety, and Personal Rights risk to persons in care. 8 Facility will provide results after the water is retested. Facility will provide forms required for Self-Certification and Sampling Checklist.

Staff did not prevent a daycare child from eloping from the facility grounds
