On 5/11/26, it was observed that the medication logs on file for child 1 and 2 did not indicate the dates and times for the medications to be administered.

The most recent health assessment on file for staff person #4 was more than 24 months old (dated in March of 2023).

Staff person #1 is serving as an AGS at the facility. She was previously an aide, and does not have sufficient documentation on file to justify serving in an AGS role.

The agreements on file for children #1, #2, #3, and #4 did not include the date of the child's admission and were not signed by the operator. (CORRECTED ON SITE)

The agreement on file for child #1 was most recently signed and dated by a parent more than 6 months ago in August of 2024.

The record for staff person #2 included documentation of 9 annual training hours for the previous training year. The record for staff person #3 included documentation of 6 annual training hours for the previous training year.

According to documentation on file at the time of inspection, when staff person #1 began work in the facility, the staff record did not contain completed out-of-state clearance information as required under the CPSL, nor did it contain a copy of the request for the remaining out-of state clearance as required for provisional hiring under the CPSL. (CORRECTED ON SITE)

The adult health assessment on file for staff person 2 did not include an examination for communicable diseases and the results of that examination. It also did not include the physician's or CRNP's assessment of the person's suitability to provide child care.

The record for staff person 2 did not include a written evaluation from the previous 12 months.

The emergency contact information on file for child 3 did not include complete addresses for the child's physician or source of medical care, and for the enrolling parent's work address.

The emergency contact information on file for child 2 did not include a complete address for a listed release person.

The emergency contact information on file for children 1-4 were updated by parents in February of 2023, and then more than 6 months later in September of 2023.

Staff person 1 did not complete professional development within 90 days of hire as listed in subsections (f)1-10. The 10 hour health and safety training was completed 187 days after hire.

A written plan of daily activities was not posted for the AM program.

The agreement on file for child 1 did not specify the amount of the fee to be charged per day or per week.

The emergency contact on file for child 2 did not include the following information: -Enrolling parent's work address and work phone number -Complete address for the child's physician or source of medical care

Staff person 2 began employment without complete clearances on file. The PA state police clearance was dated 5 days after their start date, and the record did not include documentation the request had been submitted prior to the staff person's start date.

The initial health assessment on file for staff person 1 was not obtained within the 12 months prior to initial service in the child care setting (dated after start date).
