During the renewal inspection, certification representative conducted a facility walk through and the fire exit was partially obstructed by debris in the rear of the building along with a tripping/fall hazard due to a steep drop off of the cement area into the dirt and there was a metal pipe sticking out of the ground.

During the renewal inspection certification representative reviewed 5 children's files and the file for child #1 and child #3 did not include an updated health assessment.

During the renewal inspection, certification representative reviewed facility files and the policy for Shaken Baby Syndrome and Child Maltreatment did not address the identification and prevention of child maltreatment per Announcement C-22-03 that was issued on January 24,2022.

During the renewal inspection, certification representative reviewed 5 children's files, and the emergency contact forms for child #2 did not have an address for the release person(s) on the emergency contact form.

During the renewal inspection, certification representative reviewed 5 children's files and the file for child #3 did not include signed parental consent for the administration of minor first aid by facility staff.

During the renewal inspection, certification representative reviewed 3 staff files and the file for staff #1 was missing a verification of childcare experience and a verification of education.

During the renewal inspection, certification representative reviewed34 staff file and the files for staff #1 had the 1 hr health and safety update but did not contain the 10-hr. health and safety training.

On July 23, 2024, certification representative completed a complaint investigation. After speaking with staff person #1 and checking documentation at the facility, there was no documentation that the child diaper was checked every 2 hours or that the child was changed throughout the day.

During the renewal inspection on 04/24/24 the file reviewed for staff person #1 did not contain 10hr Health & Safety pre-service training within 90 days of hire.

During the initial inspection dated 10/14/2022, certification representative observed that there was no first aid kit in the main room child care space.

During initial inspection dated 10/14/2022, certification representative observed chipped paint in the back room, hallway, and main room in 4 different locations.
