On 1/18/23, at approximately 2:30pm, staff person #3 was observed caring for a group of 10 mixed-age children (3 infants, 2 young toddlers and 5 older toddlers) in the 1st floor childcare space. Staff person #3 verified the ages of the children in care. There was no other staff/facility person in the facility. On 3/24/23, the facility did not develop written staffing plan.

On 2/17/23, at approximately 2:00pm, staff person #12 was observed caring for a group of 9 mixed-age children (5 young toddlers and 4 young school-age) in the 1st floor childcare space. Staff person #2 verified the ages of the children in care.

A review of records on 2/17/23 revealed that on 1/25/23, at approximately 1:21pm, that one staff person #3 was caring for 4 children all day with no other staff person present.

On 2/17/23, at approximately 2:00pm, a school age child was observed in the 2nd floor classroom alone and unattended with no staff supervision.

An infant (child #1) was observed sleeping face down in rest equipment. Staff person #1 acknowledged placing the child face down to sleep based upon the parent's written preference. There was no medical documentation from a physician for the infant not to sleep in this position.

On 1/18/23, at approximately 2:30pm, an aide was observed caring for a group of 10 mixed age children (3 infants, 2 young toddlers and 5 older toddlers) without being minimally supervised by an assistance group supervisor. (staff #3)

Staff person #3 verified the ages of the children in care. There was no other staff/facility person in the facility.
