The lidded trash cans in the older toddler room and the preschool room bathroom were missing the foot pedal leaving a metal rod exposed.
Staff #1 had a health assessment on 10/5/23 and not again until 2/24/26, a gap of greater than 24 months.
The current health assessment for staff #1 indicates a communicable disease with no additional information included.
The health assessment for staff #3 did not include sufficient documentation of the physician's assessment of the staff's suitability to provide child care.

There was no cot labeled for child #1 who was in attendance and asleep in the classroom.
Staff #1 had a health assessment dated 3/28/25 (see code sheet for start date). Although there was another health assessment on file, it was more than 12 months prior to the start date.

It was observed that a toilet in the girl's bathroom downstairs was not working and would not flush.
It was observed in the preschool room that medication and a medication log were both present for child #2. However, the medication present was an Epi Pen containing .15mg of medication, but the medication log, giving signed parental consent for administration of the medication and required dosage information, said Avi Q and did not specify the dosage. Staff explained that an Avi Q used to be present for child #2, but had since been sent home and replaced by the Epi Pen. Although an Avi Q was not observed at the facility for child #2, the medical documentation collected on file indicated that the previous Avi Q would have only contained .1mg of medication and an increase in the dosage was needed. No signed parental consent for the administration of the Epi Pen containing .15mg of medication was found on file.
Emergency contact form for child #1 did not contain the health insurance policy number.
Staff person #1, see code sheet for hire date, did not complete 12 clock hours of child care training. Only 10 hours of documented training was observed on file completed between 12/18/22-12/18/23.
It was observed in the temporary outdoor play space used by school age and preschool children that the lights hanging down under the pavilion area were unprotected glass bulbs making them a visible hazard. It was observed in the temporary outdoor play space used by infants and toddlers that a metal pipe was sticking up out of the ground and an AC unit was left accessible to children in that space, creating a visible hazard. It was observed in the school age room that a box of matches were stored in a draw accessible to children.
Files for staff person #1 did not contain a health assessment that included an examination for communicable diseases or information verifying that they are capable of providing adequate care to children.
“Core safety” marks citations about supervision and staff ratios, discipline and caregiver conduct, background checks, child protection and safe sleep, the topics our score weights most. Pennsylvania’s records don’t grade violations by severity. “Follow-up” marks a violation re-cited at a follow-up visit, counted once. How scores work.
Complaint investigations and reported incidents
Pennsylvania’s search lists complaint investigations in which the state found at least one violation (“verified complaints”). It does not say what a complaint alleged, or whether every violation relates to it. Incident investigations look into something that happened at the center; self-reported violations are ones the center reported to the state itself.
