At the time of unannounced inspection on 7/11/25, file of staff persons #3 and #4 did not include two written, nonfamily references from individuals attesting to the person's suitability to serve as a facility person.

During the inspection on 5/1/2025, video footage from 4/25/2025 was reviewed. It was observed that Staff Person #4, who holds the qualification of an aide, was caring for children alone during nap time in the Toddler Room and was not supervised by a staff person qualified at minimum as an assistant group supervisor.

During the inspection conducted on 5/1/2025, the director submitted DHS-approved documentation used for tracking child supervision. Upon review of the documentation, the following discrepancies were identified: - Staff Person #2 was recorded as present and supervising children on 4/25/2025 from 12 PM to 5:30 PM in Preschool room. However, records and staff confirmation indicate that Staff Person #2 left the facility at 2:00 PM and was not supervising children for the remainder of the day. - Staff Person #3 was listed as supervising children in the Toddler Room from 1:30 PM to 2:30 PM. However, it was determined that during nap time, Staff Person #3 was not present in the Toddler Room and was not actively supervising the children.

During the inspection on 5/1/2025, file of staff person #1 did not include staff health assessment.

During the inspection on 5/6/25 video footage from 5/1/25 was reviewed. On the video footage from 5/1/25 from 6 AM to 7:40 AM, staff person #1, whose file did not include health and safety training as well as Pediatric First aid and CPR training, was observed caring for children without being supervised by an assistant group supervisor, who has completed the required trainings.

During the inspection conducted on 5/6/2025, file of staff person #1 did not include health and safety training as well as Pediatric First Aid and CPR training.

During the inspection on 5/1/2025, individual records for Child #1 and Child #2 had not been established and provided for review.

During the inspection on 5/1/2025, video footage from 4/25/2025 was reviewed. The footage of a nap time in Toddler room showed at least 14 children, ranging in age from young toddlers to preschoolers, with staff person #1. One child, identifiable by a red shirt, was seen leaving the room without supervision. Older school age child #4 was walking in the facility without being supervised. Additional footage from the office area on 4/25/2025 showed Child #1 and Child #2 leaving the office area without staff supervision. The staff present in the office could not see, hear, or assess the children's movements. Furthermore, during the complaint investigation, it was reported that Child #3 did exited the building unsupervised and went to the facility's outdoor gated play space without being seen by Staff Person #1 or Staff Person #2. Both staff were unaware of Child #3's whereabouts until they observed the child outside attempting to squeeze between the poles of the playground gate.

At the time of unannounced inspection on 4/25/25, the door to Mechanical room with HVAC and water heater was not locked and accessible to children. Mechanical room is located in the main hallway children use to enter their classrooms. Non complaint since 2/27/25.

At the time of unannounced inspection on 4/25/25 observed old carpet and buggy stroller obstructing exit door located near the bathroom and Preschool room.

At the time of unannounced inspection on 4/25/25, health report of staff person #1 did not include the results of initial tuberculin skin tests, x-rays or other medical documentation necessary to confirm freedom from communicable tuberculosis.

At the time of unannounced inspection on 4/25/25, health report of staff person #1 did not include an examination for communicable diseases and the physician's or CRNP's assessment of the person's suitability to provide child care.

At the time of unannounced inspection on 4/25/25, files of children #1 and #3 did not include an initial health report within 60 days of enrollment. Non complaint since 2/27/25.

At the time of the inspection on 2/25/25 observed staff person #1 leaving children unattended in the office area to open the side door. Staff person #1 is not able to see, hear or assess children during the trips to open the door. On 2/27/25 observed child #1 being alone on the cot in Infant/Toddler Room. No staff present. Facility was cited for supervision on 12/17/24 and 1/30/25.

At the time of renewal inspection observed fire extinguisher on the kids table in Toddler room. Fire extinguisher was accessible to children. At the time of renewal inspection observed that a first aid kit in Preschool room didn't contain medical tape.

At the time of renewal inspection observed spots of peeling paint in the Toddler room near the door, orange chairs and a black stroller.

At the time of renewal inspection observed ripped changing table mat with foam exposed in Infant/toddler room.

At the time of renewal inspection, file of child #2 did not include an updated health report required at least every 12 months. Child health report on file was from 9/20/23.

At the time of renewal inspection, immunization record of child #6 did not include MMR, Varicella, and Hepatitis A immunization recommended by ACIP. Two doses of Hepatitis B, one dose of DTAP, and one dose of Pneumococcal were missing. A letter of exemption was not observed in the file of child #6.

At the time of the inspection on 1/28/25 observed staff person #1 caring for 4 children unsupervised at around 7 am. Staff person #1 had not completed the following pre-service training required prior to caring for children unsupervised: Health and safety training and Pediatric First aid and CPR. The facility's acceptable plan of correction indicated the correction would be implemented on 1/13/25. On 1/28/25 an unannounced inspection was conducted. Staff person #1 still has not completed the following preservice training required prior to caring for children unsupervised: Health and safety training and Pediatric First aid and CPR. On 1/30/25 staff persons #3, #4, and #5 were working unsupervised. Staff person #3 did not completed Pediatric First aid and CPR training, file of staff person #4 did not include health and safety training, file of staff person #5 did not include Pediatric First aid and CPR as well as one hour health and safety training required by OCDEL within the prescribed timeframe which is by December 30, 2022. Non-complaint since 12/13/24.

At the time of the inspection on 1/28/25, file of staff person #1, whose provisional hire period ended on 12/19/24, did not include DHS required FBI clearance, child abuse clearance from Florida, where staff resided within the last 5 years. Staff person #1 was observed working alone with 4 children. File of staff person #2, who was driving children to school on 1/28/25, did not have current PA State police clearance, PA child abuse clearance and DHS required FBI clearance. Staff was observed being alone with 10 school age children in the van, license plate MST 2474 at around 7 am. Non-complaint since 12/13/24.

At the time of the inspection on 1/28/25 observed staff person #1, who is an aide, was working alone with children in the office area and not being supervised by a staff person qualified at minimum as an assistant group supervisor. Non-complaint since 12/13/24.

At the time of the inspection on 1/28/25 observed staff person #1 caring for 4 children unsupervised at around 7 am in the office area. No other staff was present at the facility.

At the time of the inspection on 1/28/25, provider did not provide insurance card for 2016 Ford Transit, license plate MST2474. Non-complaint since 12/13/24.

At the time of the inspection on 1/28/25 observed that children were transported to school in a 15 seats Ford T-350 Transit van with license plate MST2474. Fifteen seats van and cannot be used to transport children. Non-complaint since 12/13/24.

At the time of the inspection on 12/17/24 observed staff person #2 leaving staff person #3 out of ratio with 13 preschoolers and person #1 who is considered an older school age child in Preschool room. Staff person #3 was not able to supervise children, who were in the area around the corner of Preschool room.

At the time of the inspection on 12/17/24 observed person #1, who is considered a child, being with child #1 alone in the office area. Another staff member was in the kitchen located near the office but was not physically present and was not able to see them.

At the time of the inspection on 12/17/24 observed video footage of staff person #4, being rough with child #2 on 11/14/24. Administration provided a written record of conversation with staff person #4, who "appeared to be too rough while handling the children".

At the time of the inspection on 12/17/24, upon entering the facility, observed person #1 helping in the facility as a volunteer. Person #1 is not 16 years of age and cannot be a volunteer.

At the time of the allocated unannounced inspection on 12/13/24, toxic laundry detergent and disinfecting spray were observed in under the sink cabinet in the bathroom and were accessible to children posing a potential hazard. The label on laundry detergent and disinfecting spray stated, " Keep out of reach of children".

At the time of allocated unannounced inspection on 12/13/24 observed peeling paint near the shelves in Preschool room.

At the time of the inspection on 12/13/24, file of staff person #1, whose provisional hire period ended on 11/26/24, did not include PA state police, PA child abuse, FBI clearance, NSOR certificate, Disclosure Statement, child abuse from Florida, where staff resided within the last 5 years. Staff person #2, who is a provisional hire, was working unsupervised. File of staff person #2 did not include receipt that staff person #2 applied for FBI fingerprints and child abuse from Florida where staff resided within the next 5 years.

At the time of the inspection on 12/13/24 observed staff person #2 was working alone with children in Infant/Toddler room and not being supervised by a staff person qualified at minimum as an assistant group supervisor.

At the time of the inspection on 12/13/24 at around 7:20 observed 19 young and older school-age children being alone in school age-area watching TV and not being supervised. Staff #1 and #2 was not able to see, hear, direct and assess the activity of the children since both staff were in the office.

At the time of the inspection on 12/13/24 observed staff person #3 being in the van with 14 children ranging in age from young to older school age.

At the time of the inspection on 12/13/24, a child who is attending Kindergarten, was being transported in the van with staff person #3 and was not buckled up.

At the time of the inspection on 12/13/24 observed that insurance cards located in 2016 Ford T-350 Transit W and 2017 Ford T-350 Transit V expired on 7/1/24.

At the time of the inspection on 12/13/24 observed that children were transported to school in 2016 Ford T-350 Transit W and 2017 Ford T-350 Transit V. Both vans are 15 seats van and cannot be used to transport children.

At the time of the inspection, damaged paint was observed on the wall near cots, on the teal wall near the shelves with animals and musical instruments, on the wall near TV in Preschool room.

At the time of renewal inspection observed the side of the vanity cabinet in Fireflies room chipping easily. One sink and two toilets in kids' bathroom near Toddler room were not working.

At the time of inspection observed a big fan and strollers blocking the rear door in the facility.

During the inspection, it was observed that staff 2 had a Child Abuse Clearance (dated 6/17/17) and an FBI Clearance (dated 5/27/17) that were older than 60 months.

Children's 3 wheeled bicycles were blocking the exit door near the toddler's room.

During the inspection, it was observed that Child 3 did not have a Flu immunization nor a letter of exemption.

During the inspection, it was observed that the safe routes were not posted by the operator.

The operator did not have proof of notifying the local traffic authority of their location and routes around the child care facility.

During the inspection, it was observed that staff 3 had an expired CPR training certificate and did not have proof of a current CPR training certificate.

Child #3's agreement form did not have the persons designated by the parent to whom the child may be released.

During the inspection, it was observed that the policy number for the child's health insurance was missing for child 1 and child 4.

During the inspection, it was observed that the emergency contact forms were not updated in the van used to transport the children.

During the inspection it was observed that staff 1 and staff 2 only had proof of 4 hours of the required 12 hours of training needed, missing 8 hours of training.

The facility did not have a director at the time of the visit.

CHILD 2'S PHYSICAL WAS NOT SIGNED BY A PHYSICIAN, PHYSICIAN'S ASSISTANT NOR A CRNP, NOR DID IT CONTAIN THE PROFESSIONAL TITLE.

CHIILD 1'S PHYSICAL DID NOT STATE IF THE CHILD WAS FREE FROM COMMUNICABLE DISEASE AND ABLE TO PARTICIPATE IN CHILD CARE.

DURING INSPECTION STAFF 1 HAD 15 CHILDREN IN THE ROOM ALONE, THE YOUNGEST ONE WAS A YOUNG TODDLER. THE VIDEO ALSO SHOWED 18 CHILDREN IN THE ROOM WITH ONLY STAFF 2.
