At the time of inspection facility did not have fire drill testing every 30 days and facility did not have proof of purchase of the interconnected system on file at the facility.

Facility did not have an emergency plan that addressed continuity of operations.

At the time of inspection staff ran from the door and hid in the classroom when agent knocked on the door. Agent had to enter facility through church entrance which was not locked or secured from the public.

At the time of inspection, the Preschool classroom did not have the daily schedule posted.

At the time of inspection none of the classroom, which children were observed, had emergency contact forms in the classrooms.

While conducting a compliant investigation on 4/20/23 Cert Rep was informed by staff person# 1 that on 4/18/23 the facility was having an issue staffing and classrooms had to be combined with mixed ages in result caused a ratio issue.

While conducting a complaint inspection on 4/20/23 Cert Rep was informed by staff person#1 that on 4/18/23 due to staffing issues; staff person#2 was supervising classroom# 1 and 2 from the hallway. Children were asleep in the dark in an unsupervised classroom.

While conducting a complaint investigation on 4/20/23 a child sleeping in a bouncy chair was observed due to combining the infant and younger toddlers in one classroom. Cert Rep was also informed by staff person#1 that on 4/18/23 was sleeping in a bouncy chair due to combining ages during nap time.

While conducting a complaint inspection on 4/20/23 it was observed the outside play area was not safe toddlers for children to play. The equipment is not age appropriate and the height does not have the required protective covering.

During the renewal inspection on 2/23/2022 emergency medication was observed at the facility for child #3. The operator did not have the parent provide written consent for administration of medication.
