Based on the information obtained during the investigation, there is sufficient evidence to validate that an infant was given diced pieces of hot dog that contained traces of pork resulting in an infant having a delayed allergic reaction the following day at home and had to be hospitalized.
The last fire inspection was conducted on December 30,2024, which a current one has not been set up.
Two out of six children's records did not have an address for the emergency contact. This was corrected when the director received the information.
A changing mat was observed to be torn making it unable to be properly sanitized. This was corrected when the changing mat was replaced with a new one. Several ceiling fans were observed with dust on the blades and vents. This was corrected when the fans and vents were cleaned.
The 18-2-year-old room was observed with lunch being served on a bare table for the children.
The opening room was observed this morning with the TV on and three of the children were under the age of two.
When reviewing the handbook, numbers: 9,18,19,20 were not available for review. This was corrected when the director added the standards to the handbook.
