At the time of the inspection the fence located at the back of the playground had gaps due to missing pieces of the fence panel pieces.
It was found through the course of investigation that a child was unaccounted for and left outside for between 5-10 minutes.
A caregiver was not adequately supervised to ensure that caregivers fulfilled their responsibilities.
A caregiver shamed children for accidents. A caregiver would crumble student work in front of them and throw it in the trash when done incorrectly.
A caregiver yelled at children. A caregiver told other children to hit a child.
