Based on information obtained through the investigation it was determined that the operation did not communicate effectively when a child was injured on a field trip. The extent of an injury obtained was not relayed in a way that allowed the parent to make an informed decision on whether the child needed medical attention or not.

At the time of the inspection a barrier to contain loose fill had a tear in it that was big enough to stick a pen in with sharp plastic edges. A play structure had rotting wood on a portion that supports a platform. The structure is not in use but still accessible to the children.
