Upon reviewing the video, it was found that a child did not wash their hands after having their pull-up was changed.
When reviewing video footage, it was determined that a caregiver did not wash her hands after taking off her gloves when assisting with diaper changing.
There was a caregiver without a background check in the room with direct access to children.
The operational policies did not have a written policy for parents to breastfeed their infant.
It was determined in the investigation that proper supervision was not utilized when caregivers failed to ensure that all children were in the group before a transition from the playground, and a child was left on the playground for 6-8 minutes.
It was determined that care that was consistent with a child's habits was not provided when staff did not implement any protocols for a child who had a history of eloping during transitions from the playground.
It was determined that a lapse in judgment occurred when a caregiver discovered a child had been left outside. Instead of immediately retrieving the child, the caregiver called for the child from the door, then reentered the building to address another caregiver before finally going back outside to bring the child in.
During the course of the investigation, it was determined that a child was left behind in their classroom as the rest of the children were evacuated during a fire alarm.

During the investigation, it was stated the victim was seen playing on play structure over 32 inches.
At time of inspection, a caregiver stated they had seven children when they had eight. This was corrected at inspection.
A caregiver did not interact with a child in a positive manner when they pulled a child's hair to prevent them from crossing the street.

Based on the information gathered, it was determined that the child was not supervised when the child opened the first aid kit and removed the scissors, and placed the scissors in the child's mouth. The child walked out to the playground and slid down the slide with the scissor still in the child's mouth before another caregiver on another playground noticed the scissors. Note: The rubber gloves and safety pins were also found on the playground from the same first aid kit.
Based on the information gathered the first aid kit was accessible to the children in a classroom that resulted in a child having access to open the kit.
At the time of inspection there was one infant in the 12 to 17 month old classroom with their head covered. THis was corrected when the child's head was uncovered.
The infant gate was broken in the 12 to 18 month old classroom. This was corrected when the gate was replaced with a new gate. The floors in the toddler classrooms were dirty.
At the time of inspection the caregivers in direct care of children were using their cell phones.
At the time of inspection the changing table in the 12 to 18 month was not disinfected after diaper change.
At the time of inspection the fire exit was blocked in the two year old classroom.
At the time of inspection the caregivers did not know who was in charge of the operation. This was corrected during the inspection.
At the time of inspection the caregiver in the toddler room did not sanitize the diaper changing table. This was corrected when the caregiver sanitized the diaper changing table.
At the time of inspection the child's hands were not washed with soap. This was corrected when the caregiver, washed the child's hand with soap.
It was determined that a child was left unsupervised in the classroom.
Three carpets throughout the building were dirty and need to be cleaned. This was corrected when the carpets were removed from the classrooms.
The diaper changing mat in the 12 to 18 month old classroom was ripped and could no longer be disinfected. This was corrected when the changing mat was replaced with a new mat.
Two shelves were not secured to the wall in the toddler classroom. This was corrected when the shelves were removed from the classroom.
One infant feeding schedule was expired at the time of inspection.
At the time of inspection there was a fan accessible to the children in care. This was corrected when the fan was removed for the classroom.
At the time of inspection there were very few toys in the toddler classroom. This was corrected when the toys were returned to the classroom.
At the time of inspection, there were not any napkins under the snack served in the four year old classroom. This was corrected when the napkins were placed under the snack being served.
Based on the information gathered the caregiver in the 12 to 17 month old classroom left a child on the changing table while walking across the classroom to the trash can.
Based on the information gathered one shelf was not secured in the 18 to 24 month old classroom, when it fell on top of a child.

The paint behind the toilet in the three year old classroom was peeling.

One caregiver did not complete the 24 hours of preservice hours within the first 90 days of employment.
At the time of inspection there were two infants napping in restrictive devices. This was corrected when the children were placed in the crib to nap.
A review of the Employees Who Left Employment in 2021 data from February 23, 2022 revealed that the center had not reported the number of employees who left employment in 2021 by February 22, 2022 as required.
