It has been determined through investigation that children were disallowed from joining a holiday event due to behavior the day before.

Through investigation it was determined caregiver pulled child into a classroom in a manner deficient of minimum standards.

Based on the information obtained, it was determined a two year old child was left unsupervised on the outdoor playground.

Based on the information obtained, it was determined a parent was not notified immediately of a situation where the child was placed at risk.

During this investigation, it was determined the operation did not report a two year old was left unsupervised on the outdoor playground within the required two day time frame.

The sign in and out log was reviewed in Classroom 3A and showed two of the children present were not signed in. Note: This was corrected during inspection.

The operation was unable to provide documentation showing a fire inspection was completed in the year 2022.

I observed children from the 2-3 year old classroom walking down the hallway to the restroom. The children were unsupervised as the caregiver responsible for them was in the classroom.

A review of the Employees Who Left Employment in 2021 data from February 23, 2022, revealed that the center had not reported the number of employees who left employment in 2021 by February 22, 2022, as required.
