A caregiver in the Violet Pre-K classroom did not have a background check submitted under the operation. They previously had an eligible background check under the operation but it had been inactivated. Note: The caregiver was sent home during the inspection.
Supervision was not compliant as a child was left unattended on the playground for approximately 15 minutes when a class transitioned back inside from the playground.
The operational policies did not include the criteria used to determine when extreme weather conditions pose a significant health risk that prohibits or limits outdoor play.
Based on information gathered in the investigation it was determined the caregiver of a child left unattended in the classroom did not notice the child was missing when they got to the playground and during their time outside. They did not realize the child was missing from the group until they were alerted by someone picking up a child. This substantiates that the caregiver did not know which children (or how many) they were responsible for. Information gathered indicated transition protocols were not followed.
Supervision was not in compliance when a child was left alone unattended inside the classroom when the classroom transitioned outside to the playground.
