Four records were revirewed and 3 are missing required training hours for previous full training year.

During inspection a video was playing in the infant room. It was turned off.

Infant feeding sheets for infants not eating table food were not available.

During inspection the sheets in the cribs were not observed to be tight fitting as required.

Six employee records were reviewed. The records were not maintained and training records as well as personnel information was incomplete.

During inspection, the infant room had a child that was more than 18 months older than the youngest infant in the group.

During an inspection, a second mop bucket was observed accessible to children that also had cleaning chemicals in reach.

During inspection, two infants under 12m were observed sleeping in a crib covered by a blanket. The blankets were remoived.

During the inspection a mop bucket was observed full of water accessible to children.

During the inispection, several toilets were found to have not been sanitized in several days.

During inspection, two changing mats were found to have cracks in them.

During the inspection 5 personnel records were observed, out of these 3 of the records did not have the required 24 hours of annual training.
